Appointments are made by calling our office.  You can request a specific counselor or be assigned based on the type of counseling you seek.  Our goal is to connect you with a clinician who specializes in your area of concern.

LifeWork is a registered provider with many major insurance companies.  When you call for an appointment you will be asked to provide insurance information to determine your level of coverage, deductible and co-pay information.

Please arrive early on your first visit to fill out the necessary information required to facilitate your counseling.  You can fill these forms out prior to your first visit by clicking: LifeWork Information Form and printing the information form.
